What is Prevention Support in GP Practices?

The Prevention & Support Services team work across the county and comprise of a number of roles which empower people to take control of their health and wellbeing through giving them time; focusing on ‘what matters to me’; taking a holistic approach to an individual’s health and wellbeing and connecting people to diverse community groups and statutory services for practical and emotional support.  
Who is Preventative Support in GP Practices for?

People with one or more long-term health conditions; people who need support with their mental health; people who are lonely or isolated; and those complex social issues which affect their wellbeing.
How do I access this service?

People / patients are referred in surgeries by either the reception team or by the doctors.

Call us on 0300 330 5488 or email enquiries@norfolkandwaveneymind.org.uk where you will be met by a Recovery Triage Operator who will provide you with further information about this service.
