What is Primary Care?

The Primary Care Services team work across the county and comprise of a number of roles which empower people to take control of their health and wellbeing through giving them time; focusing on ‘what matters to me’; taking a holistic approach to an individual’s health and wellbeing and connecting people to diverse community groups and statutory services for practical and emotional support.  
Who is Primary Care for?

People with one or more long-term health conditions; people who need support with their mental health; people who are lonely or isolated; and those complex social issues which affect their wellbeing.

How do I access this service?

To access Norfolk and Waveney Mind’s Primary Care Service you need to call or visit your local GP Surgery and ask to be referred to a Enhanced Recovery Worker.

How do I find out more about this service?

Call us on 0300 330 5488 option 1 or email rictriage@norfolkandwaveneymind.org.uk where you will be met by a Recovery Triage Operator who will provide you with further information about this service.
