What are Social Community Groups?

The service creates opportunities for people to connect with each other in a community group setting or over zoom; where individuals feel able to discuss their issues privately with staff who can support them to find the appropriate help, as well as enjoying and learning in the social aspect of the group environment. 
 
Who are Social Community Groups for?

The service supports a wide range of people including, but not limited to: people with one or more long-term health conditions; people who need support with their mental health; people who are lonely or isolated; and those with complex social issues which affect their wellbeing. 

 

How do I access this service?

The services can be accessed by the following teams and services:
Personal Budget support groups ( Heacham and King’s Lynn Social Activity Hubs ) are referred into by the service that has assessed for, and granted the Personal Budget, normally (but not exclusively)

  • Adult Social Services
  • Adult Mental Health Social Services
  • Self-funded

Call us on 0300 330 5488 option 1 or email rictriage@norfolkandwaveneymind.org.uk where you will be met by a Recovery Triage Operator who will provide you with further information about this service.
