What is West Crisis Community Support?

West Crisis Community Support is an admission prevention service, supporting individuals and NSFT teams to keep people well in their own homes and the community rather than admission to hospital.

West Community Crisis Intervention service ( WCCI) is commissioned by Norfolk and Suffolk Foundation Trust (NSFT)  

The West Community Crisis Intervention service operates across West Norfolk.  This service works together with NSFT, Crisis resolution home team (CRHT) and the Community Mental Health teams within Chatterton House, Thurlow House, Samphire ward and across West Norfolk.

 

 

Who is West Crisis Community Support for?

West Crisis Community Support is available to those aged 18 +.registered with a GP practice in West Norfolk who have been referred by NSFT services.

West Community Crisis Intervention service provides follow up support to those people referred by the NSFT teams who do not require Secondary or Tertiary Mental Health Interventions, or who are waiting for treatment (on a waiting list) from wellbeing or community mental health teams, but may still pose a risk to self.

 


 

 

How do I access this service?

Support is offered for 2-6 weeks via face-to-face meetings; this can be in a cafĂ© or community setting that is most comfortable and convenient for the client. Support can also be offered via telephone or Zoom if this is more suitable to the client.  

The West Community Crisis Intervention service offers: 
  • signposting, recommendations and referrals to other services who may be able to offer longer term, more tailored support.  
  • A listening ear, however we are not a counselling/therapy service.  
  • Support to access the community (shopping, acc​essing social activities) 
  • Regular supportive communication with assigned WCCI recovery worker. 
  • Support with follow appointments and assessments, communications with referrers.
